Me, Myself and I: Developing the “I” Care Plan for Your Residents

The goal of this session is to introduce a more person-directed care planning approach that is in line with culture change.

Resident choice is paramount to new regulations. This session introduces participants to the I-Care Plan; a person-centered approach to care. The differences between medically-based care plans and person-centered plans are reviewed; examples of an I-Care Plan are offered. Steps to developing person-centered care plans are reviewed including exploratory questions to uncover personal preferences. Care plan meetings are discussed including participant roles and adjusting the meeting from the “traditional” to person-directed. Often, providers want to honor resident choice but have fears of injury, risk, or liability. Key is balancing quality care/safety with choice which, while potentially risky, improves QOL. To address risk, the steps of Care Planning for Resident Choice will be reviewed.
