Veterans: Care Tool Available for Long-Term Services and Supports

The Veterans Administration released its Guide to Long Term Care, which provides a "Shared Decision Making" approach to long term care decisions. It includes a Shared Decision Making Worksheet to help older Veterans with care needs and their families figure out what long term care services or settings may best meet their needs now or in the future.  There's also a Caregiver Self-Assessment to help caregivers understand how the Veterans Administration can help them care for the Veteran.  

The Veterans Administration (VA) released its Guide to Long Term Care, which provides information about long-term care options – home and community based, and residential care as well as the listings of nursing homes for veterans by state.

The guide provides consumers information on how often they can receive the services, eligibility criteria, the services offered and testimonials from veterans that are using the services.    

Some of the home and community-based services that are included in the guide are adult day services, homemaker and home health aide services, hospice and palliative care services and Programs of All Inclusive Care for the Elderly (PACE).
