Medicaid Health Home: New Integrated Care Resource Center Resource

The Integrated Care Resource Center (ICRC) is sharing emerging best practices in a new resource called, "At-a-Glance" Guide to Federal Medicaid Authorities Useful in Restructuring Medicaid Health Care Delivery or Payment technical assistance tool. 

This resource summarizes federal authorities that may be useful in restructuring Medicaid health care delivery or payment, and that can be exercised through State Plan Amendments or waivers. It also highlights the flexibilities and limitations of each authority. Health Homes have the potential to improve access to home and community-based services.

The new Medicaid Health Home State Plan Option allows states to develop health homes to improve the care coordination for beneficiaries with chronic conditions. Health Homes consists of  primary, acute, behavioral, and long-term and social service providers. Many states are interested in the health home model. 

What is the Integrated Care Resource Center?

The ICRC is a national initiative of CMS to help states improve the quality and cost-effectiveness of care for Medicaid's high-need, high-cost beneficiaries.  The state technical assistance activities are coordinated by Mathematica Policy Research and the Center for Health Care Strategies.
